04-22-21 Family and Student On-Site Vaccine Clinic Information

Dear Williamsville Central School District Families,

On Friday, April 30, 2021, the District is hosting an on-site COVID-19 vaccination clinic starting at 3 p.m. at Maple East Elementary School cafeteria for students who are 16-years of age, or older, as of April 30, 2021, and all parents/guardians who have children in the District.

The vaccine that will be administered is the Pfizer 2-dose vaccine.

Parent/Guardian and Student Information

To register yourself or your child(ren), parents will need to log in to their parent WITS account and schedule a time in the WITS Appointment Calendar.

If you do not have a WITS account, please call the Parent Help Desk at 626-8058.

Please note that if you sign-up and schedule an appointment, the COVID-19 consent packet must be printed and completed prior to your or your child’s vaccination appointment.  The consent form is available once you schedule your appointment(s) in WITS.

Parents must consent and accompany their child(ren) to the vaccine appointment.

As a reminder, your second vaccine appointment will be at the same time as your first scheduled appointment on Friday, May 21, 2021.  For example, if you are scheduled to receive your first dose on Friday, April 30, 2021 at 3:30 p.m., you will be automatically scheduled to receive your second dose on Friday, May 21, 2021 at 3:30 p.m.

When you arrive at Maple East Elementary School to receive the vaccine, please park in the lots located on the side or back of the school. Please do not park in the bus loop.
