05-24-21 Student on-site vaccination clinic information for June

Dear Williamsville Central School District Families,
On Friday, June 4, 2021, the District is hosting an on-site COVID-19 vaccination clinic starting at 3 p.m. at Maple East Elementary School for students who are 12-years of age or older, as of June 4, 2021.
The vaccine that will be administered is the Pfizer 2-dose vaccine.
To register your child(ren), parents will need to log in to their parent WITS account, select the month of June on the calendar, and schedule a time in the WITS Appointment Calendar.
If you do not have a WITS account, please call the Parent Help Desk at 626-8058.
Please note that if you sign-up and schedule an appointment, the COVID-19 consent packet must be printed and completed prior to your child’s vaccination appointment. The consent form is available once you schedule your appointment(s) in WITS.
Parents must consent and accompany their child(ren) to the vaccination appointment. Please also bring a copy of child’s photo ID and insurance card.
As a reminder, the second vaccination appointment will be at the same time as your first scheduled appointment on Friday, June 25, 2021. For example, if you are scheduled to receive your first dose on Friday, June 4, 2021 at 3:30 p.m., you will be automatically scheduled to receive your second dose on Friday, June 25, 2021 at 3:30 p.m.
When you arrive at Maple East Elementary School to receive the vaccine, please park in the lots located on the side or back of the school. Please do not park in the bus loop.
