12-03-21 Update to Families Regarding 5-11 COVID-19 Vaccination Clinic

Dear Williamsville Central School District Families,

In partnership with the Erie County Department of Health (ECDOH), on Friday, December 17, 2021, the District is hosting an on-site COVID-19 vaccination clinic at Forest Elementary School for students ages 5-11.  This clinic will take place from 3 p.m. – 7 p.m. in the school’s gymnasium.  Please enter through door 12, which is the entrance right off of North Forest Road.

The vaccine that will be administered is the Pfizer 2-dose vaccine.

To register your child(ren), you will need to click this link and register through ECDOH: ECDOH-WCSD COVID-19 Vaccination Clinic.

At this time, there are a limited number of appointments available. Upon request, parents accompanying their child(ren) will also be able to receive a COVID-19 booster dose.

Please note that if you sign-up and schedule an appointment, parents must accompany their child(ren) to the vaccine appointment and bring proper identification for them and their child, which includes:

  • driver’s license
  • insurance card
  • birth certificate

As a reminder, the second vaccine appointment will be at the same time as your first scheduled appointment on Friday, January 7, 2022.  For example, if you are scheduled to receive your first dose on Friday, December 17, 2021 at 3:30 p.m., you will automatically be scheduled to receive your second dose on Friday, January 7, 2022 at 3:30 p.m.
